- Proof of payment of the DGT "Tasa 4.4" (currently around 21.30 euros). You can pay this online or at the Jefatura.
- A recent passport-sized photograph (32x26 mm) if you are submitting the request in person at the Jefatura.
- In case of theft, it is highly recommended to provide a copy of the police report (denuncia) filed at the Guardia Civil or local police station.
